Membership and Service Agreement

1. Membership Benefits The Member is entitled to the following benefits as part of their membership:

  • Access to the services included in the specific membership tier.

  • Priority booking and discounts on additional services.

  • 15% off on services, 10% off on bundles, 20% on injectables (neurotoxins, PRP, dermal fillers) special prices on workshops and events (details provided periodically).

  • Other benefits as outlined in the specific membership tier.

  • Memberships and Services are non refundable.

3. Payment Methods Accepted payment methods include:

  • Credit/Debit Card (Auto-Debit Option Available)

  • Bank Transfer

  • Other options as specified by Modern Wellness

4. Term and Renewal

  • Membership will automatically renew monthly unless canceled by either party at least 7 days prior to the renewal date.

  • Prepaid three months Memberships won’t renew automatically.

  • If active members don’t use their monthly services, they can carry them over up to 180 days.

  • Memberships are non transferrable.

5. Cancellation Policy

  • Members may cancel their membership by providing written notice or cancel themself on the client’s Zenoti portal to at least 7 days before next billing cycle.

  • If a Member does not use their allocated services before cancellation those services will be forfeited.

6. Appointment Booking, No Show and Rescheduling

  • Appointments must be scheduled in advance and are subject to availability.

  • Members must provide at least 24 hours notice for cancellations or rescheduling. Failure to do so may result in a 50% cancellation fee.

  • No-shows will incur a charge equal to 100% of the treatment price.

  • For members, failure to attend a scheduled appointment without notice will result in the loss of that month’s service credit under the membership.

7. Code of Conduct Members agree to:

  • Treat all staff and other members with respect.

  • Adhere to the policies and guidelines of the wellness center

  • Refrain from disruptive or inappropriate behavior while on premises.

8. Liability Waiver The Member acknowledges that the services provided by Modern Wellness are not a substitute for professional medical care. The Member agrees to release and hold harmless the company and its staff from any liability arising from participation in services or use of facilities.

9. Amendments to Terms Modern Wellness reserves the right to update or modify this Agreement. Members will be notified of any changes in writing at least 30 days prior to the changes taking effect.

10. Confidentiality Modern Wellness values the privacy of its Members. All personal information will be kept confidential and used solely for the purpose of providing services.

11. Gift Cards & Prepaid Cards Gift cards are non-refundable, valid for one year from the date of purchase, and may be transferred to another person. Gift cards cannot be redeemed for cash.
